If you don’t smile as much as you would like to because of chipped or crooked teeth, gaps, discoloring, mismatched crowns, an uneven gum line—whatever is holding you back, Dr. Vicki Borowski is here to help. One of DFW’s leading cosmetic and general dentists, she has been giving her patients countless reasons to smile and feel happy for over 25 years. We recently spoke with Dr. Borowski to find out what’s new in her office, and the latest trends in dentistry.

Are people looking for something different when they come in for a smile makeover consultation today compared to a few years ago?

Dr. Borowski: More people are choosing a shade for veneers and crowns that appears bright, fresh and healthy, but not so blazing white that it advertises the fact they’ve had work done. Quality porcelain veneers can last for decades. What looks fantastic at age 30 might not look as good at 50. Choosing a more natural shade helps ensure your smile won’t look out of place over time.

So is everyone going for a smile that isn’t megawatt bright?

Dr. Borowski: Not everyone. It’s a personal choice. Some patients, especially younger ones, want a dazzling bright smile. Whatever the case may be, patients need to make a decision they will be happy with.

What’s one of the biggest concerns people express when they initially see you for a cosmetic consultation?

Dr. Borowski: It gets back to wanting a smile that looks natural. They want to be reassured their new smile will not look fake. Unfortunately, we’ve all seen examples of bad dentistry. There’s just no need for teeth to end up looking like this. The porcelains we have today can mimic natural tooth structure. It is also important to use a high quality lab with technicians that are skilled in making veneers look natural.

We’re hearing more about thin veneers that don’t require the teeth to be ground down. Do porcelain veneers always need a lot of prep work?

Dr. Borowski: Not necessarily. I try to conserve as much of the natural tooth as possible. Times have changed and techniques have improved. Now the objective is to be as conservative as possible while taking into account the patients’ aesthetic goals.

Why do you believe tooth conservation is important?

Dr. Borowski: We know conservative dental restorations mean healthier teeth in the long term. I believe in taking a balanced approach to tooth conservation, and in creating a smile that is beautiful as well as healthy and functional over time. Only when a tooth has lots of old restorations or is extremely dark is it necessary to aggressively reshape the tooth for a porcelain veneer.

How do you correct crooked teeth and still conserve as much of the tooth as possible?

Dr. Borowski: Straightening slightly crooked teeth before putting veneers on them allows me to take a more conservative approach. I often use 6 Month Smiles for adults who have teeth that are out of position. Once the teeth have been straightened we can usually place more conservative veneers on them. In some instances, straightening teeth means the patient will require fewer veneers.

Is 6 Month Smiles different than clear aligner systems such as Invisalign?

Dr. Borowski: Yes. They consist of clear brackets and tooth-colored wires, which makes them barely perceptible. I believe they are more effective at moving teeth more quickly than clear aligners. The reality of clear aligners is that they can take longer because patients can remove them from their mouth. Patients really need to wear aligners 24 hours a day and most people don’t do this.

Are most people good candidates for 6 Month Smiles?

Dr. Borowski: Quite a few of the patients I see are good candidates. Many had braces as a child, but with time, their teeth have shifted. Often, 6 Month Smiles are the perfect solution for them, regardless of whether they want veneers are not.

How do patients decide what to have done during a smile makeover? It seems like there are lots of options.

Dr. Borowski: I sit with the patient and we talk about what their desires are in a new smile. If you basically have a healthy mouth, many times we can jump directly into the fun stuff—smile enhancement. If someone has multiple areas of decay or gum disease, we may need to do some preliminary work prior to enhancing their smile. All of this can be determined during a clinical exam.

Then, I provide options which can include teeth whitening, veneers, implants to replace missing teeth, braces and so on. We’ll also talk about old crowns that are mismatched or  have metal showing near the gumline, and look at replacing old amalgam fillings with tooth-colored fillings. We’ll go through and compare each viable option carefully.

When a smile makeover requires lots of work, is there a way to do it in steps that may help people fit it into their budget?

Dr. Borowski: Yes! We develop a master plan that can be completed in phases. It’s important to have a road map that we agree on. Often the treatment can be broken down into arches or quadrants which can be completed one at a time. Patients do need to understand that this can’t usually be done one tooth at a time.

Changing the topic, you offer both cosmetic and general dentistry using a whole health model with a focus on preventive care. What is this?

Dr. Borowski: Patients are more than just a set of teeth. Now we know there’s a link between your overall health and your oral health. It has been shown that poor oral health can negatively impact your body’s health, and vice versa. For example, periodontal disease (gum disease), which is an infection and inflammation in the oral cavity, is a serious medical problem, not just a dental problem. We’re finding the role of inflammation in the body is critical to several different diseases including cardiovascular disease and diabetes. It has even been linked to low birth weight babies. Many times doctors will require patients to have a dental cleaning before surgery to decrease the possibility of an infection caused by bacteria from the mouth.

What can patients expect during a routine exam at your office?

Dr. Borowski: During a dental hygiene visit we will screen for oral cancer, discuss sleep apnea and snoring, evaluate your teeth, gums and lips. We’ll look for signs of wear, cracks or broken teeth, and for changes since your last visit. Finally, we will clean your teeth.

They can start fabricating a crown right away,” Dr. Borowski said. “It’s faster than taking impressions and patients love the fact that they don’t have to deal with messy, goopy impressions.”

DIY Cosmetic Dentistry? Beware!

Sometimes DIY to save money can be a good idea. Shine your own shoes, paint a room, fix a doorknob, why not give it a try? But cosmetic dentistry isn’t only risky business, it can destroy your smile. According to Dr. Borowski, one of the most concerning new online offers is a DIY version of the Snap On Smile.

“The manufacturer sends people a kit to take an impression of their own teeth without a dentist. Using the impression provided, they create something called a press on veneer and ship it to you. I think calling them veneers is misleading. The product is essentially a tooth-colored grill.

“There are several problems with these DIY dental appliances. First, taking a correct dental impression is not for amateurs. It’s not easy to get as precise an impression as a dentist with years of experience. A poor impression will produce unsatisfactory results, including gum irritation and bleeding.

“Second, odd, fake-looking teeth are unflattering and can actually draw negative attention to your smile. People don’t see you, instead, they focus on your teeth to try to figure out what’s strange about them.

“Last but not least, some people may be tempted to use this device to mask and ignore serious dental problems. If you have a misalignment, they may further damage your bite. If your teeth are broken or have fallen out, you may suffer from periodontal disease which can  lead to heart disease and other problems. Covering them with press on veneers won’t correct the problem.”
